Important Health Coverage Tax Document
Form 1095-C, Employer-Provided Health Insurance Offer and Coverage provides important tax information about your offer of health coverage by Rensselaer Polytechnic Institute. If you were not a full-time employee or covered individual at any point during the calendar year, you do not receive a Form 1095-C. To obtain or request your Form 1095-C you can:
Active Faculty & Staff
- Access your 1095-C in your Benelogic account:
- Using your RCS credentials to log into your rpi.benelogic.com account
- Click on the Tools tab
- Click the File Cabinet tab
- Click on the 1095-C of the tax year you would like to view and save/print the PDF for your records
- Request via email at hrmail@rpi.edu
